 ESSER Data Collection Instructions

Data Submission is due by 5 p.m. April 15, 2024

Instructions:

Video Walkthrough (15 min)

Thank you for completing the ESSER Data Collection Annual Report.

You may have more than one data collection file in your folder.  If you have two files, they are different. You MUST submit each one separately.

Do NOT change the file name.

1. Fill in your answers along row 11 (Blue cells) with data that comes from your District.

Type any comments in row 12 if necessary.

2. Save your completed file for your own records.

3. Upload your completed Data Collection file into the Google Form linked below.

IMPORTANT:

Prepopulated cell data / gray cells: Some cells are filled in with data your school/district previously submitted. These cells appear gray.

Data Entry software: Excel is being used for data collection. If your district does NOT have Excel, contact us before starting. Converting the file is likely to cause errors even if it is Google Sheets.

    ESSER Self-Assessment Monitoring

    Monitoring Overview

    Monitoring the use of funds is a grant requirement and benefit to awardees. OPI is committed to collaborating with Local Educational Agencies (LEAs) to use the monitoring process as an opportunity to move beyond compliance. The Office of Public Instruction’s goal in monitoring is to help build the capacity of school districts so that they are aware of federal funding requirements, have the ability to self-assess against the requirements of the grants, and understand how they can utilize funding under the grants to improve services and secure resources.

    Self-Assessment Monitoring process closed November 30, 2023.

    1.  Select the ESSER Self-Assessment Monitoring Overview (red button to the right) which will open a document (pdf) describing the purpose, requirements, directions, and section details.
    2. Select the ESSER Self-Assessment Monitoring Template (red button to the right) (Excel) and follow the instructions on the first tab.

    *Note: Ensure that you have answered every question by scrolling through each tab before submission.
