Meal Counting and Claiming Self-Review

The annual Meal Counting and Claiming Self-Review and the second Afterschool Snack Program Review are due to be completed by February 1, 2011 (as applicable).  These forms can be downloaded at http://www.opi.mt.gov/Programs/SchoolPrograms/School_Nutrition/index.html#p7GPc1_6

For meal counting and claiming, go to the website, click on "School Programs Forms and Guidance" and look under "Meal Counting and Claiming" for the onsite review of meal counting and claiming procedures.

For the afterschool snack program, go to the website, click on "Afterschool Snack Program Packet" and look for "Page 10 - Afterschool Snack Program Review."

Please retain the completed reviews with your files.  A copy does not need to be sent to School Nutrition Programs.
