Class 5 License procedure

I am applying for the class 5 license and I was wondering if I send page 7 to my place of education and then do they send it to you?

Also, do my 6 credits have to be from the University or can they also be from Adult Continuing Ed?



If you have completed a teaching program, then yes, please send page 7 to your college and have them send it back directly to our office.  If the six semester credits you are referring to were earned from a college that has regional accreditation, they should work for the recency requirement (because you do not have a current out of state license or if your out of state license is a life license, etc).
